Library Employee Resources: Instruction

This form is intended to collect data on all types of instruction, including workshops, and research consultations.

This form is set up for library faculty to record the significant collaborations they have had with faculty and/or students (both undergrad and graduate). By significant, we mean that the interaction reaches the level that you would record it on your annual report -- it should also have some real or expected outcomes such as a paper, digital project, event, etc.

Please fill this form out whenever you get something published, accepted, and/or presented — and feel free to submit a win for both a conference presentation acceptance and a conference presentation completed. Note that this form is in development, so if we're missing anything we could be collecting, please let us know.

The Mentorship/Scholarship committee will use this form to collect data on our scholarly outputs for the year (one of our Library KPIs for the year), and to report out on faculty wins at faculty meetings and via other communication venues.

Also PLEASE don't forget to add your publications and grants to VERSO as well!
