Incident Reporting From

This is an internal reporting form (mostly for public services) to record incidents, such as harassing behavior, property damage, students of concern, bats in the library, etc. if something happens and you think it's out of the ordinary, please consider reporting it.

Library Building/Safety Issues

Travel, P-Card, & Reimbursement

Travel pre-approval requests and all reimbursement requests should be submitted through Chrome River. Step-by-step walk-throughs on how to submit request through Chrome River along with a FAQ can be found here.

Purchasing Request Forms

Supplies Purchase Request Form

Place purchase requests using the University Library Purchase Request form and clicking the 'Request Service' button. It can also be accessed via ITS's support.uidaho.edu 'Service' tab and selecting Library. You must be signed in to submit the form.

IMPORTANT: If you will be requesting supplies/equipment for multiple Departments/Units (Circulation & the Studio for example) you will need to complete one form for each Department/Unit.

Supplies Purchasing Process
  1. Request submissions using the form will be routed to Brittni and Samm automatically
  2. They will determine if any additional approvals are necessary, and then obtain them for you
  3. If any additional justification or information is sought, they will reach out to you directly
  4. Requests will be compiled into a purchasing queue and orders will be placed each Monday- If you have an immediate need for supplies/equipment, please reach out to Brittni directly.

If you are making your own purchases and they are coming from a certified vendor (like Amazon or Staples), you need to make those purchases through Jaggaer.

Librarian Materials Purchasing

  1. (Preferred method) Purchase materials directly through Alma with Rialto. PowerPoint on how to search and order through Rialto.
    You can change the shelving location or leave notes for tech services about the item in the shopping cart:

  2. Request Form (formerly Liaison Book Purchase Request): Place purchase requests for materials (books, DVDs, etc) using this form: https://forms.office.com/r/SKiT0nufgw. Rami, Samantha, and Lyn are able to access the form to purchase the requested item(s).

How to Make Reservations in the Calendar

  1. Login to LibApps and go to LibCal
  2. Select Events in top bar
  3. Choose Calendar you want to add an event to
  4. Click Add New Event
  5. Select Date and Time and add event information
  6. Select Room and Campus. (For First Floor Classroom select the "business hours" option) 

Revoking Patron Library Privileges Procedure

We have the opportunity to temporarily or permanently revoke physical library privileges from students or community members.

This process will work as follows:

  • A patron violates our new (very general) Library Code of Conduct
  • A library employee files a conduct report and sends an email to the Dean, Rami, and Samm describing the incident
  • The Dean will make the decision to revoke physical library privileges (or not) and communicate this action to the patron
  • Privileges may be revoked for one week, two weeks, a month, etc.
    • During this time, if the patron attempts to visit the library, we can ask them to leave or call Security
  • Privileges may only be reinstated following a personal meeting with the Dean to discuss the terms of reinstatement
